At LAKE ARROWHEAD HOME HEALTHCARE, LLC, we care for our patients’ health and wellbeing and provide personalized medical home care attention that will get them feeling better again.
SKILLED NURSING
Our skilled nursing services help patients regain their independence and recover from injury or illness. Our wide range of nursing services include: Post Hospitalization Assessment, Wound Care, IV Infusion Therapy, Medication Management, Ostomy Care, Continence Care, Nutritional Consultation, Diabetic Care and Teaching, Cardiac Care, Disease Management and Patient/Family/Caregiver Education.
REHABILITATION THERAPY
Our Rehabilitation Therapy services (Physical Therapy, Occupational Therapy and Speech Therapy) work with patients one-on-one in their own environment. Providing services to the patient's needs and goals. With mobility, motor skills and lifestyle changes while being able to perform activities of daily living.
MEDICAL SOCIAL WORKER
Our Medical Social Workers understand the stress and uncertainty that comes with recovery and can provide compassionate care with emotional support for patients and their families. We can help improve the quality of life with educating patients and their families about community resources, counseling, crisis intervention and advance directives. Our main priority is ensuring all the patient’s needs, both physically and mentally, are met throughout their treatment.
HOME HEALTH AIDE
Our Home Health Aides (CHHAs) assist patients in their homes with personal hygiene, activities of daily living (dressing, bathing, oral hygiene, grooming), and other personal assistance duties (with mobility and meal preparation), as well as coordinating care with our multidisciplinary team. These services are only available in conjunction with our nursing or rehabilitation services.
PRIVATE DUTY SERVICES
These services are not typically covered by insurance plans and are usually privately paid. Please call for pricing.
Our Private Duty Services can assist with Shopping/Errands, Light house keeping, Meal Preparation, Personal Care, Respite Care and Companionship.
